Using health data to flip the inverse care law in polymyalgia rheumatica

Polymyalgia rheumatica (PMR) is a common inflammatory musculoskeletal disease affecting older people. It is treated in primary care with long-term corticosteroids (prednisolone). This can cause serious health problems including diabetes, hypertension, bone fracture and infection. The average patient with PMR is at significantly higher risk of all of these, compared to someone without PMR. Currently PMR is treated with a “one size fits all” paradigm, but patients who are already face health problems due to social deprivation are likely to do much worse. The “inverse care law”, first proposed in 1971, describes the unfair situation whereby patients most affected by socio-economic deprivation are least able to access care.

<p>To redress the balance, we need to target this patient group for intensive preventive therapy in a multimodal approach including pharmacological and non-pharmacological interventions to prevent complications in those at highest risk. First, we need to define this highest-risk group so that services can be designed to be inclusive of everyone.&nbsp;</p> <p>Currently, there is no algorithm (prognostic model) to identify which patients with PMR are at highest risk of serious health effects from steroids.&nbsp;</p> <p>The PhD will use existing routinely-collected health datasets and statistical modelling to develop prognostic models to predict one or more of the four serious steroid-related health problems: diabetes, hypertension, bone fracture and infection, in patients with PMR. These will then be validated in a cohort of patients with PMR within UK Biobank and the potential value of additional information available in this research cohort will be explored. A degree in biological sciences, dentistry, medicine, midwifery, nursing, psychology or a good honours degree (2:1 or above) in a subject relevant to the research topic. A Masters degree is desirable but not essential.

Applicants whose first language is not English must provide evidence that their English language is sufficient to meet the specific demands of their study. The Faculty of Medicine and Health minimum requirements in IELTS and TOEFL tests for PhD, MSc, MPhil, MD are: &bull; British Council IELTS - score of 6.5 overall, with no element less than 6.0 &bull; TOEFL iBT - overall score of 92 with the listening and reading element no less than 21, writing element no less than 22 and the speaking element no less than 23.

<p>The University of Leeds is providing generous matched funding, supplementing the NIHR Leeds BRC award, to allow us to appoint one further non-clinical PhD Fellow who can select from 18 BRC related projects. Each PhD will be registered at the University of Leeds and we welcome applications from individuals, irrespective of their background, who can evidence great potential and commitment.</p> <p>The&nbsp;scholarship will provide academic&nbsp;fees at the UK rate, as well as a&nbsp;stipend of &pound;18,622 (based on the 2023/24 stipend rate, pro-rata for part time study) for up to three years (full time study, five years part time) and is available for&nbsp;UK applicants&nbsp;only. Due to limited funding we can only consider applicants eligible to pay academic fees at the UK fee rate.</p>
